I wanted to bring something to your attention immediately. This morning, one of your drivers, with tag number 6AM2228, was driving very erratically – swerving across lanes and nearly causing an accident. I had to take evasive action to avoid a collision. I then pulled him over to ensure he was alright, and I observed he was using his phone at the time. I believe this situation requires immediate attention to ensure the safety of all drivers. I have a video recording of the incident and would be happy to send it to you.
